Membership price must be paid in full at the time of enrollment. Membership plan is non-refundable even if the plan is not used by the participant in the given benefit year. Membership plan is not a dental insurance plan but a dental discount plan honored only at DentAllon Dentistry. Membership plan cannot be used: In conjunction with another dental plan or dental insurance; In combination with CareCredit; For referrals to specialist; For hospitalization or hospital charges of any kind; For cost of dental care which are covered under automobile or medical insurance; For services of injuries covered under workers compensation.
